For members of the public: 

  • Together for Sutton is the London Borough of Sutton’s information, advice, support and advocacy service and may be able to assist with your query. You can contact them Monday to Friday, 9am to 5pm, on 020 8254 2616. 
  • The Sutton Information Hub is an easy-to-search catalogue of services and groups in the local area.
  • If you are looking for care and support for you or someone else, we encourage you to use our Eligibility Checker to see if you/they are likely to qualify for care and support paid for fully or partially by the Council and help you to understand your options.
  • If you or a family member needs support to manage daily activities at home more independently or safely, you can request an Occupational Therapy (OT) assessment.
  • To report a Safeguarding concern, such as the neglect or abuse of yourself or another adult, please complete our Safeguarding Referral Form (Public) which will help us to gather the information we need and respond as quickly and effectively as possible. 
  • The main telephone number to contact LBS Adult Social Care remains as 020 8770 5000, effective Monday to Friday, 9am - 5pm.

 

For professionals (excluding the emergency services*):

  • We request all referrals from professionals to be submitted via the Professional Referral Online Form. This form covers Care Act Assessments, Carers Assessments, Occupational Therapy Assessments, Vision and Hearing Assessments and Section 117 Aftercare. 
  • To report a safeguarding concern, please complete our Safeguarding Referral Form (Professional) 
  • If you encounter any issues with the above, you can contact us by phone, Monday to Friday, 9am - 5pm on 020 8770 5000

*Police, Fire, Ambulance and NHS Hospitals, we will continue to receive your referrals via the referral point email.
